This Regulation prescribes the application form for qualification in the public procurement procedure. The application for qualification in the public procurement procedure contains data on: 1) the date and time of submission of the application for qualification; 2) status of application for qualification; 3) to the applicant for qualification; 4) public procurement, namely: - the name of the ordering party, - the procedure code, - the description of the subject of the public procurement, - the type of subject, - the type of procedure, - the type of phase; 5) conditions for participation in the procedure. The Annex is also part of this text (form samples).
