National Strategy and Action Plan for Antimicrobial Resistance Containment in Bangladesh (2023-2028).

The updated National Strategy and Action Plan for Antimicrobial Resistance (AMR) Containment provides for the action plan to respond to the growing threat of AMR in Bangladesh. The earlier plan achieved some progress in awareness and surveillance, but implementation remained slow because of inadequate financing, shortages of trained personnel, weak coordination and the absence of a national monitoring and evaluation framework. The revised plan therefore adopts a comprehensive One Health approach, covering human, animal, aquatic and environmental health. Its overarching goal is to minimise the development and spread of AMR while ensuring that antimicrobials remain effective and are used appropriately. The plan is organised around eight strategic priorities: (1) establishing integrated AMR surveillance and strengthening laboratory capacity; (2) promoting rational antimicrobial use and antimicrobial stewardship; (3) improving sanitation, hygiene and infection prevention and control; (4) increasing stakeholder awareness and engagement; (5) strengthening multisectoral coordination; (6) promoting AMR research and innovation; (7) reducing environmental spread of AMR; and (8) strengthening international collaboration and partnerships. Key actions include strengthening reference and sentinel laboratories, improving AMR/antimicrobial-use data collection and sharing, developing treatment and antibiotic-use guidelines, implementing stewardship programmes, strengthening IPC, water and sanitation, biosafety and biosecurity, improving professional training and public awareness, strengthening legal and regulatory frameworks, improving waste management, supporting research and alternatives to antimicrobials, and building national and international networks. Activities are prioritised into short, medium and long-term measures, beginning with governance, coordination, communication, legal frameworks and professional development and progressing towards institutionalised surveillance, stewardship and stronger institutional capacity. The implementation is intended to be multisectoral, involving various government institutions and departments, together with development partners. The updated plan introduces a formal monitoring and evaluation framework and a theory of change approach so that activities can be prioritised, progress periodically assessed and implementation measured through defined outcomes and indicators. This is intended to address weaknesses of the previous NAP and to make AMR containment a more sustained, institutionalised national programme across all One Health sectors.
